I have read & understood the below information and by signing this form I recognize this to be my (electronic) signature in agreement.
I understand the agreement to this contract? Yes, No or Modify Contract (please submit your request in writing to KatieYavuzCTT@gmail.com).
The State of Colorado recognizes that the Religious sector and State are separate per the First Constitutional rights. The State of Colorado recognizes that we have many solemnized options.
I understand we have hired Rev. Katie Yavuz to officiate our Wedding for her talents in: Public Speaking, Creative Writing Skills for our Ceremony, and her Omnipresence with Source as an Ordained Minister, Custom Wedding Options/Ceremonies and Products/Services.
What is your Cancellation Policy?
Please remember Rev. Katie Yavuz has a 48 hour cancellation policy. There is a $50 charge per meeting for less than a 48 hour cancellation or no show. There is a $70 charge per venue/group sessions (2+ people), Officiant Services, R.S. Pastoral Practitioner meetings/sessions, Meditations, House Blessings or Guest Speaking that requires two or more hours after 6 p.m.
A secured reservation for your rehearsal/wedding ceremony on Paypal.com is required prior to any work to be started on your ceremony.
If you cancel at any point into our committed Wedding Ceremony agreement any written ceremonies or content that has been started and/or furnished to you the client and you cancel without good cause, your quoted price will be invalid. You will be billed at an hourly rate of $147/per hour for services such as writing your ceremony any emails or phone calls not returned. Any travel or products being completed for your wedding will have the same rate as stated in the Wedding Catalog for Season 2016 (to be updated).
If you refuse to pay for the “wording of your ceremony” you are not authorized to use it. You must submit proof that you did not use it by audio-video and submit a written form of a ceremony you chose to use after canceling services with Colorful Conscious Weddings/Rev. Katie Yavuz. You must submit this proof within one week after your ceremony to avoid additional charges or collections. This protects copy right laws established per federal laws to the author Rev. Katie Yavuz.
If you need to cancel or reschedule an appointment, please do so by phone at: 720-270-8161 only. This cancellation policy is for the courtesy of your Officiant/Religious Science Pastoral Practitioner. If you are scheduled for an appointment, and don’t show, this leaves your Officiant/R.S. Pastoral Practitioner without the option for another appointment/booking including accruing further expenses.
If the ceremony includes a sand ceremony, unity candle, or any other special feature, the client is responsible for furnishing all equipment needed. Unless stated in payment option with or without a kit. For a list of current *Optional Ceremony prices please refer to Wedding Catalog April 2014 mail jpeg.
If the client wishes to have the Officiant attend the rehearsal, it needs to be explicitly added to the package, and an additional fee will apply.
Can I get a refund?
Unfortunately, with the nature of services and meetings/sessions other costs are accrued prior to your rehearsal/wedding venue date. If deemed necessary a partial refund maybe issue upon agreement of both parties,
I understand emergency’s come up; you will be required to submit in writing your request for refund. Refunds will only be issued within the first 60 days of the Date Issued Invoice (perhttp://www.Paypal.com policies).
All security deposits are non-refundable unless the Officiant cancels the wedding. If you’re not satisfied with your services you may donate or give additional sessions to a friend or relative. No refunds will be made after a meeting/session/rehearsal /wedding that has already taken place.
The agreement of the Officiant to perform the ceremony is subject to proven detention by accidents, riots, strikes, epidemics, acts of God, or any other legitimate conditions beyond their control. If such circumstances arise, all reasonable efforts will be made by the Officiant to find a replacement. Should the Officiant be unable to secure a replacement to perform the ceremony, client shall receive a full refund. Client agrees that in all circumstances, Officiant liability shall be exclusively limited to an amount equal to the performance fee and that Officiant shall not be liable for indirect or consequential damages arising from any breach of contract.
Officiant agrees to arrive at the wedding location listed on the contract at the agreed time. Client understands that they must adhere to these times but Officiant does understand weddings do not always go as planned and will accommodate for weddings that start later than scheduled, as time permits. Officiant will allow for a 1.5 hour block of time per wedding to give plenty of time for unexpected delays.
After a gift certificate has been purchased no refunds will be given; however, all gift certificates are transferable. I will allow an extended time frame up to a year on expired date of certificate for gift certificates to be honored. (Example: gift certificate was dated to expire on July 12, 2013; you would have until July 12, 2014 to use the gift certificate.)
Weddings/Memorials/Group Meditations: I also agree to allow my picture to be taken with Rev. Katie and I allow her to post it online or other documentation(s). I may opt out by stating so under “Comments” up above.
Client understands that it is the client’s responsibility to acquire a valid marriage license in the state where the wedding ceremony will take place and the marriage license must be on the premises when the ceremony is performed. Client understands that failure to have these legal documents means that the wedding Officiant cannot legally perform the ceremony and is under no obligation to do so. The Officiant may at his/her discretion perform a symbolic ceremony, which will have no legal merit, and perform another ceremony in private for the couple once the legal documents are presented, for an additional fee. Client will consult with the local court or city clerk to find the exact requirements for the client’s area (website stated above on page 1).
I am required to keep all of our communication confidential, according to the Colorful Conscious Wedding’s Code of Ethics & Guidelines. Colorado Statutes require me to report to the appropriate agency(s) any acts of child abuse if you choose to disclose and shared this in our meeting/session /ceremony. In addition, I will also report information or experiences revealed to me regarding instances of elder abuse or any intent to cause physical harm or death to another person.
“White Lace” Adult Wedding (services): Pastoral Counseling is required to establish a “safe word” and know each individuals comforts, discomforts, to set safe boundaries within the covenant of a faithful marriage. This helps to provide trust with both parties with the comfort of love, passion and a good sense of knowing the other person desires to explore sexuality with their partner in a safe and loving manner. 10% will be donated from each “White Lace” wedding ceremony to help raise rape awareness. (Both parties must provide proof of being over the age 18 years old and sign a waiver that they are both consenting adults additional professional signature may be needed for unique special needs/requests.)
